Golden Valley utilizes a variety of forms to assist its Membership. To better serve you, below are electronic copies of our commonly used forms. You can submit a completed form through this site, via email (if PDF) or by dropping it off with any of our Member Service Representatives. If there is a form that you require and it cannot be found here, please contact us, and we will be happy to assist you.
If you are a Business and wish to establish electric service, please complete the Business Membership Application and return it to one of our Member Service Representatives. If you are a new Business, you will need to provide a copy of your business license along with a completed W-9 Form. If you wish to have an employee conduct business on your behalf, you will also need to complete a Business Designee form. *All business requests must be in writing.
